Now Hiring: Hybrid Nurse Practitioner | Full-Time

Sacramento, CA | Innovative Patient-Centered Clinical Team

Make a meaningful impact in the lives of patients navigating complex chronic conditions. If you're an experienced Nurse Practitioner who values autonomy, deep patient relationships, and interdisciplinary care—this could be your next role.

This full-time hybrid position supports a growing clinical care team that partners with dialysis centers in the Greater Sacramento area. With a blend of in-person and remote work, this role offers flexibility, clinical depth, and the chance to help transform care for high-needs patients.

Key Responsibilities:

  • Deliver in-person patient visits 3–4 days/week in Sacramento clinics
  • Conduct 1–2 days/week of remote care via telehealth and administrative tasks
  • Provide comprehensive clinical assessments and patient education
  • Develop and manage individualized care plans for patients with complex needs
  • Collaborate with integrated care teams to ensure coordinated, high-quality care
  • Serve as a consistent, trusted clinical presence for patients across settings

Schedule:

Monday–Friday | *Self-managed*

Qualifications:

  • Licensed Nurse Practitioner in California
  • DEA license (or eligible to obtain within 90 days)
  • BLS or CPR certification
  • Minimum 2 years' experience as a Nurse Practitioner
  • Strongly preferred: background in Internal Medicine, Family Medicine, Hospitalist, Cardiology, Nephrology, or Case Management
  • Proficient in managing chronic, complex conditions in adult patients
  • Reliable personal transportation; must reside in or near Monterey County
  • EMR experience preferred (Cerner familiarity a plus)
