This full-time hybrid position supports a growing clinical care team that partners with dialysis centers in the Greater Sacramento area. With a blend of in-person and remote work, this role offers flexibility, clinical depth, and the chance to help transform care for high-needs patients.
Key Responsibilities:
- Deliver in-person patient visits 3–4 days/week in Sacramento clinics
- Conduct 1–2 days/week of remote care via telehealth and administrative tasks
- Provide comprehensive clinical assessments and patient education
- Develop and manage individualized care plans for patients with complex needs
- Collaborate with integrated care teams to ensure coordinated, high-quality care
- Serve as a consistent, trusted clinical presence for patients across settings
Schedule:
Monday–Friday | *Self-managed*
Qualifications:
- Licensed Nurse Practitioner in California
- DEA license (or eligible to obtain within 90 days)
- BLS or CPR certification
- Minimum 2 years' experience as a Nurse Practitioner
- Strongly preferred: background in Internal Medicine, Family Medicine, Hospitalist, Cardiology, Nephrology, or Case Management
- Proficient in managing chronic, complex conditions in adult patients
- Reliable personal transportation; must reside in or near Monterey County
- EMR experience preferred (Cerner familiarity a plus)
